AppointMe assessment reports include self-assessments, official assessments, and referee coaching assessments. They are completed by the officials that are are appointed to a fixture. Once completed and approved (if required), a PDF of the assessment report will become available to download.

Roles That Can Complete AppointMe Assessment Reports
AppointMe assessment reports are available to the Referee, Referee Assessor, and Referee Mentor officials that are appointed to a fixture. The role that is assigned to each official will determine which report will be available for them to complete.
If you are a:

Referee, Assistant Referee 1 (AR1), Assistant Referee 2 (AR2), or 4th Official (4thO)
- The Referee Self-Assessment Report will be available to complete for yourself

Referee Assessor
- The Official Assessment Report will be available to complete for each other official appointed to the fixture with the role of:
- Referee
- Assistant Referee 1
- Assistant Referee 2
- 4th Official
- The Official Assessment Report will be available to complete for each other official appointed to the fixture with the role of:

Referee Mentor/Referee Coach
- The Referee Coaching Report will be available to complete for the Referee of the fixture
Completing AppointMe Assessment Reports
The process for completing AppointMe Assessment Reports is the same, regardless of which role you have been appointed to for the fixture.
1. Open AppointMe by Navigating to www.sporty.co.nz/AppointMe
2. Click on View My Appointments
3. Click on the panel for the fixture that needs an assessment report to be filled out
4. Click on the hyperlink next to the report name to open the report in a new tab.
The link will be next to the name of the person that it is being filled out for i.e. next to your own name for a Self-Assessment or next to the Referees name if you are a Referee Assessor completing an Official Assessment Report for that Referee
- Referees will see: Start Referee Self-Assessment Report
- Referee Assessors will see: Start Official Assessment Report
- Referee Mentors/Coaches will see: Start Referee Coaching Report
5. Fill out the report form - some details about the fixture official and fixture details will be automatically populated.
Note: the report form's questions and auto populated details will vary between reports.
- Clicking Save will save your current progress and allow you to open the report again later to pick up where you left off. The report can be opened by following the same hyperlink which will be updated from Start to Edit Draft.
- Clicking Submit will submit the final version of the report and the report link will be updated with a new link that will generate a PDF of the report.
Viewing Assessment Report PDFs
Once the PDF is available to be viewed, a hyperlink that opens the PDF will become available next to the name of the person that completed the report. See below for when each PDF will become available:

Referee Self-Assessment Report/Referee Coaching Report
- As soon as the Referee Self-Assessment Report has been submitted, the PDF report will become available.

Official Assessment Report

Once a federation administrator has reviewed and approved the Official Assessment Report that was submitted by the Referee Assessor, the PDF will become available to the official that the assessment report was filled out for.

Approving Official Assessment Reports
A federation administrator is required to review and approve Official Assessment Reports before the PDF becomes available to the Referee that it was completed for.
1. Open Online Registrations and open the Official Assessment Report form's database.
2. Open the form to review the assessment report that was submitted by the Referee Assessor.
The report includes fields that specify who the report was filled out by and who it was filled out for.
3. Click the + button to approve the report and make the PDF available to the Referee who was being assessed.
