You must complete this form before your licence can be approved. You can complete it right after submitting your MSNZ Competition Licence application, or edit it later through your profile.
If you can't complete the medical assessment at this time, click Save - this lets you come back and edit the form from your profile. Once the form is submitted, it will be sent to MSNZ for review.
Assessment Type - Actions To Take
The assessment type determines which set of questions you need to complete. It's auto-populated from your MSNZ Competition Licence application, based on your licence grade.
Junior Medical Declaration - Junior M Grade
- Download and print the Junior Medical Declaration form from the link and complete it with your GP.
- Take a photo of the completed form, then upload it to the medical assessment form.
National Medical Declaration - National Race 1/2, National Rally 1/2
- Complete the list of questions on the digital application form. If you answer any question with "Yes," add supporting comments in the section provided.
International Medical Card - International Grades
- Download and print the medical declaration form from the link and complete it with your GP.
- Take a photo of the completed form, then upload it to the medical assessment form.
Junior Fitness Appraisal - National Race 1/2 Junior, National Rally 1/2 Junior
- Download and print the Fitness Appraisal form from the link and complete it with your P.E. teacher.
- Take a photo of the completed form, then upload it to the medical assessment form.
Fitness Declaration - ClubSport
- Complete the Disability Declaration.
Submitting the Application
A medical assessment can be edited, and the form saved, as many times as you need while you gather information. Once you click Submit, the application will be sent to MSNZ for review.
Click the pencil icon next to your application to edit it. Once you've completed all requirements, make sure you check the declaration box so MSNZ knows the application is ready to be processed.
Renew
A medical declaration can be renewed if you're reapplying for an international medical card. The medical card has a 12-month expiry but can be completed at any time.
Other medical assessments are only required when completing a new licence and don't need to be renewed.

Auto Approval
The medical assessment can be auto-approved if:
- There is a current approved medical card, or
- All medical questions are answered "No" on a National Medical Declaration or Fitness Declaration, or
- There have been no changes since the previous approved National Medical Declaration assessment, and it is still current.
MSNZ Review
MSNZ will review the medical assessment and approve or decline the request. A next review date will be set, and MSNZ will decide whether an additional fee is required to cover any medical expenses.
Once approved, this updates the Medical Assessment Status on your MSNZ Competition Licence form.
